How to Create a Demo Qualification Form

Capture enough buying context to prepare a relevant demo and route early research appropriately.

How to Create a Demo Qualification Form

A demo form should help a sales team prepare, not create unnecessary friction. Ask for the context that changes the conversation: the problem, users, timing, fit, and decision path.

Step 1: Start with business context

Collect business email, company, role, team size, and the product area of interest. Use a short dropdown for the primary use case so requests can be routed consistently.

Step 2: Understand the current process

Ask how the team handles the problem today, what is not working, and what a successful outcome would look like. These answers are more useful for a tailored demo than a generic “tell us more” field alone.

Step 3: Qualify timing and decision-making

Use ranges for timeline and budget rather than demanding an exact figure. Ask who will evaluate the solution and whether procurement, security, or implementation requirements could affect the next step.

Step 4: Route and prepare

Use conditional logic to send enterprise, partner, or technical requests to the right specialist. Notify the owner with the completed context and create a suggested agenda before the meeting.

Step 5: Measure handoff quality

Track New, Qualified, Needs follow-up, Meeting booked, and Nurture. Review which questions predict a useful first meeting and remove fields that do not change routing or preparation.
