How to Create a Technical Support Request Form
Technical support requests are resolved faster when the first message includes the environment, impact, and steps needed to reproduce the issue. Keep the form structured while allowing a clear narrative.
Step 1: Identify the account and product area
Collect requester, account or workspace, product area, plan when relevant, and a short issue summary. Use account identifiers rather than asking for passwords or secrets.
Step 2: Capture reproducibility
Ask for environment, browser or device, version, expected result, actual result, and steps to reproduce. Add conditional questions for API, integration, or billing issues only when selected.
Step 3: Assess impact and urgency
Use plain-language severity options tied to examples. Ask how many users are affected, whether a workaround exists, and whether a deadline or outage is involved.
Step 4: Accept safe evidence
Allow screenshots, sanitized logs, request IDs, or short recordings when useful. Explain what must be removed, especially credentials, tokens, and personal data, and restrict access to the support team.
Step 5: Keep the requester informed
Route by product area and severity, assign an owner, and send status updates. Useful statuses include New, Triaged, Investigating, Waiting for customer, and Resolved. Capture the final cause and resolution so future requests can be answered consistently.
